Provides excellent customer service when first responding to families inquiring about PHC services. Provides thorough application assistance to ensure families overcome barriers to enrollment. Provides families with post-enrollment education to include information on access and utilization. Assists families to complete re-enrollment according to program procedure.
Partnership for Healthier Communities (PHC) is designed to identify uninsured children and adults and connect them to an appropriate and affordable source of quality healthcare services through programs like Medicaid, FAMIS, and free or low-cost medical care coverage programs.
PHC is a program of Inova Health System, with additional funding support from:
- Virginia Health Care Foundation (VHCF), - Virginia Department of Medical Assistance (DMAS), - U.S. Center for Medicare & Medicaid Services (CMS).
How does PHC work?
Through partnerships with the local school systems, governments and community outreach programs, PHC is able to identify children and adults in need of care and work with these families to provide culturally and linguistically appropriate assistance.
Services provided by PHC:
Medicaid and FAMIS Enrollment and Renewal Assistance- PHC provides one-on-one application assistance and will submit, advocate, and provide follow-up to ensure that the application is processed in a timely manner. PHC acts as a bridge between families and the Departments of Family/Social Services
Referrals to Safety-Net-Providers - PHC connects uninsured families to the best options available when they are not Medicaid or FAMIS eligible, like Inova Cares Clinic for Families and other free or low cost clinics.
Emergency Department - PHC connects patients recently discharged from Inova Fairfax Hospital Emergency Department and provides one-on-one application assistance for Medicaid/FAMIS and/or will make referrals to safety net providers.
Medicaid Workshop - PHC provides the ins and outs of how to utilize the Medicaid coverage and benefits. The importance of preventive care and a medical home.
Collaboration with Local Schools - PHC works closely with the public schools in Prince William, Fairfax and Loudoun County, and the cities of Alexandria and Manassas, to locate and assist uninsured students.
Outreach and Enrollment in the Community - PHC participates at community and school outreach events, health and resource fairs, and provides on-site enrollment assistance in the community.
Baby Shower - PHC provides education about Medicaid for Pregnant Women and Newborn, application assistance and what to expect after delivery with insurance.
Free School and Sports Physicals Events - Helping every child! PHC collaborates with community partners to provide free physicals, Medicaid/FAMIS enrollments, and safety net provider referrals on-site.
