| PRN Social Worker Case Manager I Inova Loudoun Hospital is seeking a PRN Social Worker Case Manager I to support the inpatient case management functions of progression of care and discharge planning. The PRN positions support various days to include weekends as needed. Job Duties: Provides/Evaluates biopsychosocial impact on patient plan of care. Evaluates patient ability to progress throughout the continuum of care. Works collaboratively in communication with physicians, nursing, and other members of the multi-disciplinary care team to effect timely and appropriate patient management. Working knowledge /experience in utilization management, managed care and payer issues. Psychosocial assessment and Interventions. Provides discharge planning and continuity of care for assigned patients in the acute and post-acute setting. Understands pre-acute and post-acute resources. Provides coordination of services and acts as key liaison between patient, the patient's family, and the interdisciplinary healthcare members.
